Calcium and bone growth
Bone mass and bone size are of crucial importance to determine the future risk of fractures occurring in advanced age. Calcium salt supplements administered to young girls enhance bone mineral gain but also tend to stimulate the gain in bone size in girls having a low spontaneous calcium intake. Protein supplements markedly increase plasma IGF-I in animals fed a low protein diet. This research project is aimed first to set up an animal model able to detect modification of bone mineral mass, bone size and architecture in growing rats in relation with protein and calcium intake, then to investigate the effect of calcium salts and protein intakes on bone mineral mass acquisition, in prepubertal children. Dietary protein and bone Dietary protein intake plays a role in both bone mass acquisition and bone loss. Low protein intake contributes to a marked uncoupling between bone formation and bone resorption. The research project aims at investigating how dietary proteins are influencing the coupling process between bone formation and resorption, by studying the cellular and molecular mechanisms implicated. Using experimental models of rats and mice, and various transgenic mice, we are further characterizing, in terms of skeletal site specificity, bone remodeling rate, microarchitectural alterations, bone envelop selectivity, the process leading to decreased bone strength induced by a low protein isocaloric diet. The role of protein undernutrition on bone mass, bone architecture, bone remodeling, and bone strength should be better understood. These aspects will be of major importance for the nutritional prevention and treatment of osteoporosis and of its complications. Osteoporosis epidemiology Femoral neck and vertebral fractures are associated with considerable pain and disability, and have considerable impact on public health in elderly populations. Optimal treatment and prevention of osteoporosis require knowledge on incidence, socio-economic impact and on effects of modifiable risk factors of osteoporosis. The goal of the project is to estimate the incidence of osteoporotic fractures, their risk factors and their outcome, including mortality rate. This belongs to a more than 15 years collaboration with the orthopedic department. Glucocorticoids and bone Glucocorticoid therapy is associated with accelerated bone loss and frequent low energy fracture. A paradigm of corticosteroid-induced osteoporosis is represented by patients with organ transplantation, in whom immunosuppressive therapy adds its deleterious effects to the negative influence of the underlying disease. In collaboration with the medical teams involved in transplant recipients management, systematic assessment of those patients is applied together with prevention measures under the form of bone resorption inhibitors administration. Bone microstructure During infancy and adolescence, very little is known regarding the changes in bone microarchitecture, another important determinant of bone strength, and thereby of fracture risk. There is a variety of evidences indicating that weight bearing exercise before and/or during puberty contributes to favor bone mineral mass accumulation, but the role in influencing microarchitecture is unknown. We are studying competition sport men and women at the end of the second decade of life, and comparing sports with weight bearing, non weight bearing and healthy non sport competitors of both genders. Bone microarchitecture will be assessed at the level of the distal tibia and distal radius by high resolution pQCT, a protoype lent in the frame of a European community/European space agency sponsored research project.
