L'athérosclérose et ses complications cliniques
L'athérosclérose et ses complications cliniques, telles que la thrombose artérielle, l'ischémie et l'infarctus du myocarde, du cerveau et d'autres organes, continue de représenter la majorité de la
morbidité et de la mortalité de la population adulte des pays industrialisés. L'athérosclérose est un processus progressif, multifactoriel débutant par l'accumulation dans la partie intimal de la paroi
vasculaire de cellules inflammatoires de type lymphocytes et/ou macrophages, suivi par la prolifération et la migration de cellules musculaires lisses avec déposition de matrice extra-cellulaires. Ce
processus atteint l'aorte, les artères carotidiennes, coronaires, iliaques et fémorales. Les facteurs de risque cardio-vasculaires tels que le tabagisme, le diabète, l'hypertension artérielle,
l'obésité, et l'hyperlipidémie induisent une dysfonction endothéliale qui contribue précocement aux mécanismes de l¿athérogenèse. Cependant, puisque ces facteurs de risque affectent tout le système
vasculaire, il ne peuvent à eux seuls expliquer l'atteinte focale de l'athérosclérose. Au contraire, les forces hémodynamiques imposées à la paroi vasculaire artérielle par le flux sanguin pulsatile varie
localement en fonction de la géométrie et de l'architecture du vaisseau, et sont de ce fait un bon candidat pour expliquer le processus focale lié au développement de l'athérosclérose. En effet,
plusieurs études récentes ont démontrés que l'athérosclérose se développait de façon préférentielle dans des zones de flux sanguin lent et turbulent, et presque jamais aux endroits où ce flux et rapide ou
laminaire. Il semble donc évident que les forces hémodynamiques vasculaires participent de manière importante au développement de l'athérosclérose.
La présence et l'accumulation de cellules immuno-inflammatoire, de type lymphocytes et macrophages, dans les lésions d'athérosclérose est bien documentée, mais les différents processus qui permettent le recrutement de ces cellules inflammatoires de la circulation vers la paroi vasculaires sont encore obscures et très mal compris. Dans plusieurs de recherche, nous proposons de déterminer à l'aide d'un modèle unique ex vivo d'athérosclérose l'expression et l'implication de molécules, telles que les cytokines, les chémokines, les molécules d'adhésions, dans la dysfonction endothéliale et le recrutement de cellules inflammatoires lors des premières étapes de l'athérogenèse. Nous proposons d'examiner et de déterminer le rôle des différents facteurs de risque, y compris les forces hémodynamiques, ainsi que de certains traitement (anti-diabétique, hypolipémiant) lors de ces processus initiant l'athérosclérose. Ces recherches devraient nous permettre de mieux comprendre les différents processus inflammatoires liés à l'athérogenèse, ainsi que le bénéfice de certains traitement.
