Etudes et traitement de l'hépatite C et B
Conduite d'études cliniques de traitement:
1. Traitement par interféron alpha-2a pégylé e ribavirine de patients atteints d'une hépatite C recourant après greffe de foie (étude SASL 17), avec étude de la cinétique de l'ARN viral C circulant (Investigateur principal) 2. Traitement par interféron alpha-2b pégylé en monothérapie de patients atteints d'une hépatite virale C aiguë (étude SASL 18), avec analyse de la cinétique de l'ARN viral C (sérum, PBMC) et de certains paramètres immunologiques (investigateur principal) 3. Vaccinothérapie avec protéine de l'enveloppe 2 du virus de l'hépatite C de patients avec hépatite C chronique n'ayant pas répondu au traitement antiviral standard (étude Innogenetics #916) (Co-investigateur d'une étude multicentrique européenne dirigée par le Dr. Hulstaert, Innogenetics, Ghent, Belgique) 4. Analyse de la cinétique de l'ARN viral C dans les cellules mononuclées du sang périphérique pendant traitement combiné avec interféron alpha-2a pégylé e ribavirine de patients atteints d'une hépatite C chronique, et corrélation avec d'autres paramètres virologiques et immunologiques (Etude DITTO-HCV) (Co-investigateur d'une étude multicentrique européenne dirigée par le Dr. A. Neumann, Bar-Ilan University, Israel) Organisation et gestion d'une Etude de Cohorte de patients atteints d'une hépatite C chronique à l'échelon suisse (Président) Participation au Réseau d'Excellence "Virgil", financé par la Commission Européenne, dont le but est celui de caractériser la résistance des virus de l'hépatite B et C et de la grippe aux différents antiviraux. La contribution au réseau par laboratoire dirigé par le Dr Negro consiste à: 1. participer au développement d'un système de réplication in vitro du virus de l'hépatite C subgénomique (principe du "replicon") de génotype 3a; 2. caractériser les patients atteints d'une hépatite C aiguë ou chronique et non-répondeurs aux traitement antiviraux courants par le moyen d'une méthode de quantification de l'ARN viral C en temps réel (TaqMan) dans les PBMC Etude de la pathogenèse des lésions hépatiques et extrahépatiques dans l'hépatite virale C chronique, chez les individus immunocompétents et immunodéprimés. La recherche a comme buts: ˇ l'étude de la pathogenèse de la stéatose hépatique, avec analyse des déterminants viraux (taux de multiplication virale C, génotype viral) et de l'hôte (peroxidation lipidique, résistance à l'insuline, système de la leptine) ˇ l'établissement d'un modèle de stéatose in vitro par transfection avec différents gènes du VHC ˇ l'étude du rapport entre hétérogénéité de séquence virale C et manifestations extra-hépatiques Publications du groupe Hepatitis delta virus inhibits alpha interferon signaling.
