Devenir des patients ayant séjourné aux SIC: Identification de facteurs prédictifs de la qualité de vie.
La technologie moderne des soins intensifs permet de traiter et soutenir des patients dans des états gravissimes. Toutefois, ces traitements induisent parfois des situations où les patients survivent dans des états qu'ils n'auraient peut-être pas souhaités. Ce projet vise à investiguer 1. le devenir et la qualité de vie des patients ayant séjourné aux SIC, 2. déterminer des facteurs ou événements survenant au cours du séjour aux SIC pouvant influencer la qualité de vie future de patients. 3. évaluer la capacité des patients, de leurs proches et des soignants à prédire la qualité de vie future. Un programme Qualité : · Utilisation des gazométries aux SI · Mesure de l'aPTT au lit du patient · Agitation aux SIC. L'agitation est un problème aux soins intensifs. Elle est rarement identifiée et monitorée. Les incidents sont des événements indésirables pouvant potentiellement ou réellement mettre en danger les patients. Ce projet de recherche qualitative vise à 1. quantifier, qualifier et monitorer les états d'agitations et les incidents liés survenant aux SIC, 2. implémenter des recommandations de management de ces agitations et d'investiguer les effets de telle action sur l'incidence des incidents, 3. Evaluer l'effet préventif de musique relaxante ou de la réflexologie sur l'état d'agitation (étude contrôlée, randomisée). · Test d'un nouveau dispositif pour le contrôle de la glycémie aux soins intensifs chirurgicaux Le contrôle de la glycémie s'est avérée importante pour l'amélioration de la survie des patients de soins intensifs. L'administration adéquate de l'insuline requiert des examens répétés de glycémie qui peuvent provoquer un inconfort pour les patients et un risque accru d'anémie. Ce projet consiste à tester un dispositif déjà utilisé en médecine ambulatoire pour le monitorage continu des glycémies en milieu de soins intensifs. Perception de la qualité de vie au travail des soignants des SIC Les soignants de soins intensifs sont constamment soumis à une tension importante non seulement de par la gravité des situations cliniques rencontrées, mais également l'urgence des interventions, leur agressivité de même que la charge émotionnelle majeure imposée. Cette étude par questionnaire vise à évaluer l'impact de ces facteurs sur la santé physique et psychique du personnel soignant des SIC et les ressources utilisées afin de surmonter ce stress permanent. Les résultats de cette étude devrait permettre d'appréhender des solutions pour alléger le poids porté sur les soignants dans ce milieu particulier. Taux d'apolipoprotein A-1 à l'admission aux soins intensifs et exacerbation de l'état inflammatoire. Plus de 80% des patients des SIC présentent des signes inflammatoires aigus après une chirurgie majeure, un traumatisme, une pancréatite ou un infarctus du myocarde. Il y a un intérêt accru sur la participation des dérivés lipidiques dans l'inflammation. L'apolipoproteine A1 pourrait jouer un rôle dans la modulation inflammatoire. Un taux bas d'Apo A-1 à l'admissions aux soins intensifs est possiblement associé à une agravation du SIRS.
