Les dysfonctions cardio-pulmonaires et cérébrales

Au cours de ces 10 dernières années, notre champ d'investigation comportait principalement :
1) l'étude des facteurs de risques des complications cardio-pulmonaires lors de la chirurgie cardiaque, vasculaire et thoracique (étude de cohorte et analyse multivariée);
2) l'implémentation de stratégies organo-protectrices en chirurgie majeure (par ex. : utilisation de beta-bloqueurs, de l'analgésie péridurale continue) et le traitement de l'oedème pulmonaire post-résection pulmonaire par l'inhalation d'agent beta2-mimétique;
3) les mécanismes d'adaptation circulatoire lors d'une hypoxie, d'une anémie, d'une ischémie myocardique ou d'un stress chirurgical ;
4) les interactions pharmacodynamiques entre d'une part, les agents anesthésiques et d'autre part, le bleu de méthylène ou le blocage neuraxial du système sympathique (potentialisation des effets hypnotiques).

Les dysfonctions cardio-pulmonaires et cérébrales sont appréciées par des mesures hémodynamiques invasives (pressions centrales, thermodilution, oxymétrie), l'analyse des gaz sanguins, la quantification de biomarqueurs (troponine I, SP100), des enregistrements ECG et EEG (analyse bispectrale) ainsi que par l'échocardiographie et le Doppler transcrânien.
Chez des patients qui présentaient une maladie coronarienne bi-tri-tronculaire ou une sténose sévère de la valve aortique, nous avons démontré que l'hémodilution normovolémique aigue (HNA) améliorait les conditions circulatoires (augmentation du débit cardiaque sans tachycardie) via la baisse de la viscosité sanguine tout en provoquant la libération d'érythropoiétine (EPO). De plus, parmi les patients hémodilués, nous avons observé une réduction de l'importance des lésions myocardiques ainsi que de l'incidence des arythmies et du besoin en médicaments inotropes après la chirurgie cardiaque par rapport à un groupe non-hémodilué (contrôle). Ces données cliniques ont été confirmées dans un modèle expérimental d'infarctus chez le rat où l'HNA pré-ischémique était associée à une réduction de la taille de l'infarctus (-40%), à une amélioration de la survie à 48h (83% vs 42%), ainsi qu'à une élévation importante des taux d'EPO.
D'autres études expérimentales ont révélé les mécanismes sous-jacents aux effets cardio- et neuroprotecteurs induits par l'EPO : prolifération et différenciation des cellules progénitrices érythroïdes, inhibition de l'activation de la caspase, activation de JAK-2 kinase, activation de phosphatidyl-inositol-3 kinase et phosporylation de Akt.
Mis à part l'EPO, l'infusion de Glucose-Insuline-Potassium (GIK) s'est également avérée efficace dans la prévention des lésions myocardiques d'ischémie-reperfusion via différents sites cellulaires (activation de la phospatidylinositol 3-kinase, hyperpolarisation des cardiomyocytes, Ť switch ť métabolique en faveur des glucides et augmentation des taux de glycogène cellulaire, suexpression de la voieL-Arginine-monoxide d'azote).
Dans 2 études randomisées contrôlées, nous investiguerons prochainement les effets cardioprotecteurs de l'EPO et de la solution GIK chez des patients programmés soit pour une chirurgie de remplacement valvulaire aortique soit pour des pontages aorto-coronariens.Les dysfonctions neurocognitives qui atteignent transitoirement environ 60% des patients opérés seront également investiguées dans lżétude concernant l'EPO.
