Activité d'enseignement: Problème n°1: Un nouveau-né normal

Contexte:

• Tutorial de 3h dans l'unité Pédiatrie (4ème année)
• Problème clinique: Prise en charge d’un nouveau-né normal
• Diagnostic: Nouveau-né normal
• Entités nosologiques: Anamnèse maternelle, examen clinique du nouveau-né normal, planification des soins de base et de la sortie
• Aspect(s) privilégié(s): Importance pour le nouveau-né et interprétation des tests effectués de routine pendant la grossesse. Variantes de la norme de l’examen du nouveau né et changements physiologiques au cours des premiers jours de vie. Importance des tests de dépistage et des mesures thérapeutiques effectués de routine chez le nouveau-né. Contact avec les parents et préparation à la sortie de l’hôpital

Objectifs d'enseignement:

• Les principaux facteurs anténataux pouvant affecter le nouveau-né. Pédiatrie Néonatologie
• Les paramètres principaux de la croissance et de la maturation intra-utérine. Néonatologie
• La définition du nouveau-né normal à terme. Néonatologie
• La définition de la prématurité, post-maturité, retard de croissance intra-utérin (RCIU), macrosomie, macrocéphalie, microcéphalie, des percentiles…. Néonatologie
• L’examen somatique du nouveau-né, dépistage de malformations ou d’anomalies (signes dysmorphiques, dysplasie des hanches, pied bot, hypospadias, torticolis congénital, foetopathies...). Néonatologie
• L’évaluation du développement neuromoteur (posture, tonus actif et passif, réflexes archaïques...). Néonatologie
• L’alimentation du nouveau-né (colostrum, lait maternel). Définir les risques d’hypoglycémie. Connaître les indications à un supplément alimentaire (Dextrine- Maltose, lait adapté). Pédiatrie
• La prévention de la maladie hémorragique du nouveau-né par la prophylaxie à la vitamine K. Néonatologie
• L’évaluation d’une perte de poids néonatale qui pourrait être pathologique (>10%). Néonatologie

• La définition et diagnostic différentiel des ictères du nouveau-né, les risques de cet ictère (maturations des fonctions hépatiques, hémoglobine foetale et adulte…). Néonatologie
